In order for you to receive care using Virtual Health; we will need to send you emails from time to time to schedule your appointments. Although you may not be aware of this fact, emails sent to some webmail services (e.g. Gmail, Hotmail etc.), may be stored/routed outside of Canada (for example, in the United States). Due to the fact that future emails will contain personal information about you, including your name, email and appointment information, the Freedom of Information and Protection of Privacy Act requires that we obtain your consent before we continue.
We will collect the personal information listed below to confirm your identity and enable you to access Virtual Health visits. We will only send your personal information to the personal webmail address which you have provided to us. All of the information which you provide to us, including information about the care you receive, will be kept completely confidential.
In collaboration with you, we will assess, evaluate and continuously improve our Virtual Health service, and determine if this is an effective way to provide the care you require. To support this, you may be asked to participate in a survey in the future.
Please note the PHSA Virtual Agreement is a secure online process. The reason your browser indicates the process is not secure, is that the URL applies to the entire BCW website, and not the specific Virtual Agreement webpage.
If you choose not to consent, you will not be able to access Virtual Health visits.
CONSENT: Do you voluntarily agree and give your consent for the Complex Chronic Disease Program to email your personal information to you?
