Relationship with your family doctor

Our clinic will send a letter after each clinic visit to your family doctor/referring doctor to keep them up to date about your progress and care plan. The receptionist will ask if you are still seeing the same family doctor at every clinic visit to ensure that we are sending information only to the doctor that you have authorized.

If you do not want your family doctor/referring doctor to receive these letters, please ask the receptionist to remove the doctor from your record.
