Virtual Health is a patient-centred care model focused on connecting patients, families and providers, through the use of technology to promote wellness, specialty care and improved outcomes.

The Complex Chronic Diseases Program (CCDP) is pleased to offer patients the opportunity to attend some appointments virtually using zoom.

At the time of the appointment, click the link in your email invitation to meet your provider virtually. If you experience delays joining the group, please wait until your provider initiates the meeting or admits you from the waiting room.
