The Internal Medicine Clinic provides expert advice and ongoing support to individuals with medical conditions during pregnancy, pre-pregnancy and postpartum.

Complete the referral form (PDF), including any relevant details of the patient’s medical history as well as any tests or diagnoses.​

2Send the referral form

Fax the completed referral form to 604-642-8836. 

3Post referral

The program carefully reviews all referrals. We will follow up with you to inform you whether we have accepted or declined the referral.

If we accept the referral, we will contact the patient directly to book an appointment.
