We offer long-term neurodevelopmental and medical monitoring for babies who continue to need close monitoring after discharge from the Neonatal Intensive Care Unit. Infants may be born prematurely or have medical complexity following their delivery.
Our team provides assessments of babies’ key milestones so we can help with timely referrals for community services and early developmental intervention, if needed. We see babies through this program from NICU admission to school age.
Patients need a referral from a health care provider for this program.
1Ensure the patient meets at least one of the referral criteria
We list the referral criteria on the referral form (PDF). The referring health care provider must indicate one or more reasons for referring their patients to our program.
2Complete the referral form
The referral form asks for details we need to create a new patient profile, contact the patient’s family and continue their post-NICU care. Please respond to all items on the form.
3Send the referral

4Post-referral
If the information is incomplete, we will reach out to the referring health care provider.
We will contact eligible patient families directly to confirm appointment availability and related details.
