Complex Contraception Clinic Referral

​The Complex Contraception Clinic provides care to medically complex patients over the age of 15 with a history of:
  • ​Cancer or current cancer
  • Cardiovascular disease
  • Migraines
  • Seizure disorders
  • Thrombosis/thrombophilia
  • Bleeding disorders
  • Obesity
  • Dialysis
  • Immobility/spinal cord injury
  • Uterine anomaly
  • Previous difficult IUD insertion
  • Side effect/intolerance of standard contraceptive methods 
​Our clinic does not offer sedation services for contraceptive procedures. If your patient requires sedation services or a higher level of care, please refer them to a local gynecologist. 

For patients requiring pessary care, please refer them to urogynecology.

1Complete the referral form

Please complete our referral form (PDF), along with any relevant patient information or documentation.  

2Send the referral

Please fax the completed referral form and supporting documentation to: 778-504-9805. 

3Post referral

Our nursing team reviews referrals. We triage patient based on symptomology, diagnosis and nature of disability. 

4Appointment with a provider

Once you have submitted your referral, no further action is required. We will contact patients directly to schedule an appointment.
