Diabetes in Pregnancy Referral

The Diabetes in Pregnancy Clinic provides specialized and personalized care for individuals with type 1 diabetes, type 2 diabetes, gestational diabetes and those who are risk of developing diabetes during pregnancy.
​All referrals must be made by a physician, midwife or nurse practitioner. We do not accept self-referrals from patients. Upon referral, each patient is assigned to the care of an endocrinologist.

Referral steps

1Eligibility

Patients must be pre-registered for delivery at BC Women's Hospital + Health Centre. Exceptions may be approved if required diabetes care is not available in a patients’ own community. 

The criteria for referral are as follows:
  • Type 1 diabetes
  • Type 2 diabetes
  • Gestational diabetes
  • Abnormal 50 gm screen >11.1mmol/L
  • Transfer of care
  • Asymmetrical macrosomia
  • Polyhydramnios
  • Pre-pregnancy counselling (type1, type 2, pre-diabetes) 

2Complete the referral form

Complete the referral form (PDF) in its entirety and include relevant documents as listed in the form. 

Please note: we will not process incomplete referral forms. To avoid delay,  include additional pregnancy-related documents, such as Antenatal Records 1 & 2, ultrasounds, lab results, etc. 

3Send the referral

Fax the completed referral form to 604-875-3041. 

4Post referral

The program carefully reviews all referrals. We will follow up with you to inform you whether we have accepted or declined the referral.

If we accept the referral, we will contact the patient directly to book an appointment.
