Fetal Monitoring Service Referral

Our clinic provides ongoing monitoring of a baby’s heart rate during pregnancy.
​All referrals must be made by a physician, midwife or nurse practitioner. We do not accept self-referrals from patients.

Referral steps​

1Eligibility

Please review the Antepartum Nonstress Testing and Ultrasound Surveillance Guideline (PDF) before requesting an assessment to determine if your patient is eligible for a non-stress test (NST).​

2Complete the referral form

Complete the referral form (PDF) and include relevant documents as listed in the form. 

3Send the referral

Fax the completed referral form to 604-602-8656. 

4Post referral

If we accept the referral, we will contact the patient directly to book an appointment.
