Our clinic provides prenatal procedures, including amniocentesis and chorionic villus sampling.
All referrals must be made by a physician, midwife or nurse practitioner. We do not accept self-referrals from patients.
1Eligibility
If your patient will be 35 years or older at the time of their delivery, they may be eligible for a prenatal procedure.
2Complete the referral form
Complete the correct referral form in its entirety and include relevant documents as listed in the form:
Fax the completed referral form to 604-602-8677.
4Post referral
The program carefully reviews all referrals. We will update the referring provider if we accept or decline the referral. If we accept the referral, we will contact the patient directly to book an appointment.
