Complex Chronic Diseases Program

The Complex Chronic Diseases Program (CCDP) is a provincial referral centre that provides symptom-centred care to patients across BC experiencing Myalgic Encephalmyelitis (ME/CFS), Fibromyalgia (FM), and symptoms attributed to Lyme Disease (SALD).

This service supports patients and primary care teams in the management of complex chronic diseases (CCDs). As with many chronic diseases, most patients can be diagnosed and managed in primary care. If you require advice on the diagnosis or management of people diagnosed with a CCD in primary care, you may book a physician-to-physician consult with us. Support from our clinic through a referral is also available. This webpage is intended to support health care providers in referring a patient to this program.

Service Eligibility

To be eligible to receive care at this clinic, you must meet each of the criteria below. Each referral is carefully reviewed and  assessed on an individual basis.

We do not accept self-referrals from patients. Please speak to your care provider to be referred to this clinic for care.

Criteria:

  • Gender-diverse residents of BC,
  • 19 years of age or oder,
  • With symptoms or a diagnosis of:
    • Myalgic Encephalomyelitis (ME/CFS),
    • Fibromyalgia (FM), or
    • Symptoms attributed to Lyme Disease (SALD)

Complete the referral form

Please complete the referral form in its entirety and include relevant documents as listed in the form.

 The CCDP does not accept re-referrals for patients who have participated in the program before. If your patient has participated in the CCDP before and there has been a medical change in their condition, please complete the referral form and specify that you would like to book a provider-to-provider consult.

Patients who have participated in the CCDP previously are eligible to access the CCDP community group, available as of January 2020.

Submit the referral form

Submit the completed referral form by fax to 604-875-3738.

Review of referral

All referrals are carefully reviewed by the CCDP. We will follow up with you to inform you if the referral has been accepted or declined. If the referral is accepted, we will contact the patient directly to book an appointment.
