Indigenous Complex Care Coordination

This new service compliments the long standing Indigenous Patient Liaison service. The Indigenous Complex Care team is comprised of a Community Liaison Nurse and Community Liaison Social Worker. The team takes patients and families who have complex needs and will require extra support after leaving the hospital. The two positions work together to help plan the process of returning home from hospital and follows up with the plans and progress you have made once you return home. They can help with applications and answer questions that may arise once you are back at home. This new service is slowly phasing in on the units and clinics of both BC Women's and Children's Hospitals. Services are available currently on NICU, Evergreen, and 6th floor of the Teck Acute Care Centre with more additions to come.
