Ordering Parent Information Sheets

• The Parent Information Sheets are also available in the following languages; Traditional Chinese, Simplified Chinese, Vietnamese, Korean, and Punjabi. 
• Parent Information Sheets can be ordered in all languages. Please email the BC Newborn Screening Program indicating the desired quantity (10, 25, 50, 100) and shipping address.

Ordering Blood Spot Cards

Please contact the C&W Newborn Screening Laboratory to obtain blood spot cards.

Plan ahead and allow enough time for the cards to reach you by regular mail so that you do not run out of cards.

Storage and Use of Residual Blood Spots
What if parent's do not wish to have their child's blood spot card stored once newborn screening tests are completed?
They have 2 choices.  Please have them fill out one of the following forms.

Directive to Destroy Form
Release of Leftover Form

What if parents do not wish to have their baby screened at all?
Please have them fill out the Informed Refusal Form (by Health Authority)
