We collect, use and share your personal information for these reasons:
• Your ongoing care and service
• To keep in contact with you and to help us improve our care and services
• Teaching and education (e.g. training medical students)
• Research (as permitted by FOIPPA*) and in a way that protects your privacy. You may also be asked to consent to participate in specific research projects while you are in our care
• As required by law (e.g. court order, reportable conditions, etc.)
• To know your eligibility for benefits and services
• Arrange payment
We do this under the Health Authorities Act, Hospital Act, and other legislation (e.g. Hospital Insurance Act, Community Care and Assisted Living Act, Health Act, and Mental Health Act).
*FOIPPA - BC Freedom of Information and Protection of Privacy Act.
Types of information collected:
We collect copies of records, medication information or test results from your doctor or other organizations, and information to confirm your identity and personal health number.
Where necessary, and when it is not possible to collect this directly from you, we may ask your family or friends to provide information to assist us in caring for you.
Request to access patient's file
You can request information from your child's medical file by printing the Release of Information Form and faxing it in. Please note that due to the volume of requests received, it can take four weeks or more to process release of information.
