Making a Referral
In order to better serve our patients, we have included downloadable referral forms on this site. These forms must be completed by either the Family Physician or the referring Specialist. In order to make a referral to the Provincial Medical Genetics Program, the following process needs to be followed:
For Prenatal Patients: The Prenatal Referral Form (PDF 118kb) needs to be completed in full by the referring physician. The referral then needs to be faxed to the number below along with any other required medical records (such documents are noted on the top of the referral form):

For General (Adult or Pediatric) Patients: a General Referral Form (PDF 29kb) needs to be completed by the referring physician. The form, along with any other relevant medical records, can either be mailed to the address at the top of this page or faxed to the number below.
