Confidentiality
We know confidentiality is extremely important to you. We will not disclose your HIV status without your consent. Our clinic nurse will ask your permission to obtain medical information from your family doctor. This will help us understand your medical history and current medical problems. With your permission, we will routinely send information to your family doctor to keep him or her updated with your progress and care plan. We will not disclose any information (including your HIV status) to anyone without your consent, unless we are obliged to do so by law. Information about your care and health is shared routinely among the Oak Tree Clinic multidisciplinary team, but not outside the team without your permission.
Relationship with Your Family DoctorIt is important for you that our team to keep in contact with your family doctor. With your permission, our clinic will send a letter after each clinic visit to your family doctor to keep him or her up to date about your progress and care plan.
