Preventing Heart Disease One Step at a Time
Cardiovascular disease (heart disease and stroke) is the leading cause of death for both men and women. Someone dies from heart disease or stroke every seven minutes in Canada.

Risk Factors
You might be at risk for heart disease if you have any of the following conditions:
- Dyslipidemia (high or low cholesterol)
- Hypertension (high blood pressure)
- Diabetes/Pre-Diabetes
- Family History of Heart Disease
- Smoking History
- Obesity
- Metabolic Syndrome
- Depression
- Sleep Disorder
- Physical Inactivity
- History of Gestational Diabetes (diabetes during pregnancy)
- History of Gestational Hypertension (high blood pressure during pregnancy)
Services
The program offers:
- Risk Factor Screening and Stratification
- Personalized Goal Setting
- Personlized Treatment Plan
- One hour with the Nurse Practitioner
- Ongoing Follow-up and Support as necessary
