Please note that you must be referred by a physician or a nurse practitioner to book an appointment with the Osteoporosis Program (the referral form is available below). Once we receive your referral, we will contact you to book your appointment and any tests you may need prior to your first visit.
Our referral criteria are that you have a diagnosis of osteoporosis or that you have low bone density with significant risk factors.
Referral Form - Osteoporosis Program (Please Click to Download).
Before you come to your appointment, you will need to have some tests done so we can accurately assess your risk of fracture. These may include:
Bone Density
Measures the density of bone in your skeleton. It is painless and uses a low dose of radiation.
Blood Work
Measures how well your body is using calcium and your general health. It also helps rule out medical conditions that may cause osteoporosis.
X-rays
Sometimes X-rays of the spine are needed to determine your fracture history.
We will send you a health questionnaire and a map. Please complete the questionnaire and bring it with you to your first appointment at the Osteoporosis Clinic.
Please bring your B.C. Care Card to your appointment.
